To complete the Application Form you must:
- Complete the form in full (parents or guardians).
- Ensure that all the information on the form is correct.
- Please do not complete the form in all CAPS or all lowercase, please use sentence case.

Please ensure that you have all the following documentation ready in pdf or jpeg format before you apply online
(scan individually and name accordingly):
- Copy of Birth Certificate / ID Document
- Copy of Study Permit/Refugee Permit (if foreign)
- Copy of Learner’s Latest Progress Report
- Copy of Parents/Legal Guardian’s ID Document
- Transfer Document (once available)
- Copy of Vaccination Record (Pre-Primary & Foundation Phase)
- Copy of Medical Aid card (Front and Back)
- Proof of household Income / Salary Advice 1X Month
- Proof of Residence
- Latest 3X Months Bank Statement
- Recent Colour Photo of the Learner (ID Size)
